Child Immunisation

& Health Checks

  • Childhood vaccinations are booked with the nurse and the doctor.  This is an important appointment as not only does your child receive vaccinations, this is your opportunity to speak to a doctor about your child’s development. 

  • Please bring your bluebook with you as it is important to record the vaccination as well as update the child’s development.

  • As well as the bluebook check you will be given an “Ages and Stages” questionnaire to fill out.  Again it helps us help you if this gets done.

  • Even if you think everything is going well sometimes important delays can be picked up and you may be referred for extra assessment. This doesn’t mean anything is wrong, but we find that there can be delays in getting help when we know you need it so early referrals are better.

Health and Wellness Checks For Older People

  • We offer an over 75’s annual Health & Wellness Check

  • This is done in the month or 2 before your birthday, ideally when we can coordinate it with your driving medical.

  • Please bring your driving medical form and have an optometrist fill out the vision portion ahead of time.

  • This is a aimed at keeping you well and independent by picking things up before they become a problem.

  • It incorporates a review of medications as many older people are on medications that can interact and have side effects. A memory screen – so you can tell your children to get off your case, you’re just fine, a depression screen, and is a chance to discuss the legal and financial concerns that come with getting older.

 
 

Care Plans for Chronic Disease

Care Plans

  • Care plans are your ‘roadmap’ of steps for managing your chronic illness.  It is a set of activities agreed between you, the GP nurse, and your doctor about what activities will manage your illness best.

  • You are eligible for a care plan if you have a chronic illness that is likely to last 12 months or more or is cancer.

  • With a care plan you have a prescheduled appointment 3 -6 monthly to review your progress, usually with the nurse and always with the doctor.  There are a maximum of 1 care plan and 3 reviews that can be done per year and they can only be done every 3 months.

  • Besides the benefit to improving your health, the benefit to you is these appointments are bulk billed as medicare pays a higher rebate to encourage good preventative care.

    Team Care Arrangements (and Extended Primary Care – EPC referrals)

  • If you have 3 health professionals in ‘your team’ then you have a second document called a team care arrangement.  Most people would benefit from a team care arrangement, for example if you have arthritis and need to loose weight to avoid a joint replacement we can include a dietitian or an exercise physiologist.

  • A TCA unlocks a medicare rebate of up to 5 sessions per calendar year (January – December)

  • As well as the rebate the other benefit is that the members of your treating team will be kept informed about how your condition is progressing.
